Injection site rotation: keep a clear site history
An injection site log records the location and side used for each administration. It helps you review your history while following the rotation instructions for your specific medication.
Start with your medication’s instructions
Approved injection sites depend on the medication, device, and route. There is no universal eight-site sequence or number of rest days that applies to every injection. Your product’s instructions and your clinician’s guidance determine which sites are appropriate.
For example, the patient information for semaglutide injection and tirzepatide injection describes product-specific sites and rotation instructions. A tracking app’s list of locations is not a list of approved sites for your medicine.
Use consistent names
Write the same location the same way each time. If a location is part of your prescribed instructions, include the side in its name, rather than alternating between an abbreviation, a broad body area, and a vague note. This makes later entries easier to compare.
Record the exact location in your own notes if the broader site label is not enough to distinguish spots. The name of a body region alone does not describe where within it an injection happened.
A simple site history needs four fields
- Administration date and time: when it actually happened.
- Treatment: which medication the entry belongs to.
- Location and side: where it was administered.
- Optional notes: observations or questions you want to raise with your clinician.
Use the printable injection log to put these fields beside the amount and unit. Keep separate entries when more than one treatment is administered on the same date.
Read history as history
A “last used” date is a record of an earlier event. It cannot confirm that a site is suitable today, and it cannot assess pain, irritation, or the condition of your skin. Follow your product instructions and ask your clinician about concerns.
If an entry has no site recorded, leave that gap visible rather than guessing later. An honest incomplete log is more useful than an apparently complete history made from assumptions.
